The forms listed below are available for free from the society to aid in documenting your family history. Click on the appropriate title to open the form in a PDF reader such as Adobe Acrobat Reader or Preview for the Mac. Then save the file on your computer.

You can now type directly into some of the charts once you have saved them on your computer. You can then save the completed charts and/or print them. This process, however, only works reliably using Adobe Acrobat Reader on a desktop or laptop computer. Some of the charts may work in Preview for the Mac and some may work on some tablets or phones, but for the best experience, please use Acrobat Reader.

We are currently (2015) in the process of redoing some of these charts, so please check back if what you need is not yet interactive. Thanks to volunteer Dave Krygiel for making the interactive forms.
